Blue Shield of California has teamed up with Altais® to help you schedule and complete your Annual Health Assessment. This wellness visit provides an in-depth evaluation of your overall health, helps identify any areas of concern, reviews medications and offers steps to improve your health.

What Happens During Your Annual Wellness Visit?
During your visit, your provider will review your medical history and conduct a variety of health assessments, including:
- Evaluating your lifestyle habits and identifying areas for improvement.
- Determining which preventive screenings are right for you.
- Assessing your home safety and fall risk.
- Discussing your wishes for future medical care.
- Measuring your height, weight, and body mass index

If you are a Blue Shield of California Member, you qualify for a no-cost Annual Wellness Visit.

Who is Altais Health Solutions?
Blue Shield of California has teamed up with its affiliate Altais® to help you schedule and complete your Annual Health Assessment. Altais supports you and your primary doctor in managing your care. Together, we work together to set-up, coordinate and complete your Annual Wellness Visit to help you reach and maintain your optimal health. Your visit will be conducted by a licensed nurse practitioner from one of Altais’s contracted professional entities.
Blue Shield of California is working with Altais to provide this service to eligible members. Some members may not be eligible for in-home services. Some services may not be available via virtual visits. Discuss your options with a representative when you call to schedule an appointment.
Altais is independent of Blue Shield of California and is contracted by Blue Shield to provide in-home and virtual care services.
